Testing Services are provided under a "fee-for-service", patient-directed testing model. We do not accept or bill insurance, though many insurance providers may offer reimbursement.

Health Savings Accounts, common with many High Deductible Health Plans, may be used; testing is considered a qualified medical expense.

Insurance

We do not accept or bill insurance, though many private insurance providers may reimburse you for all or part of the cost of testing based on their agreement with you. Typically, this requires downloading a reimbursement claim form from your insurance provider's website, filling it out, and submitting the form to your insurer.

Medicare Icon

Medicare / Medicaid

We do not accept or bill Medicare, Medicaid, or state-based public insurance options. Medicare, Medicaid, and state-based public insurance options require an order from a qualified provider to receive reimbursement. Patient-directed testing, which comprises our entire menu, do not qualify for submission to these insurance options. Patients who utilize these insurance options, including Medicare supplemental insurance options, will be responsible for the payment of any services ordered.

Private Insurance Submissions

While euverita does not submit claims to insurance, we can provide most of the information you will need to successfully submit a claim on your own behalf.

Private Insurance Submission Information

While euverita does not accept or process insurance claims, many private insurance companies are willing to reimburse patients for some or all of the cost of the laboratory tests we provide as part of their "out-of-network" claims process. Medicare, Medicare Supplemental, Medicaid, and State-sponsored insurance plans will not pay for direct-to-patient testing.
